Frequently Asked Questions

You may ask us any type of question about Nexxus care. You can ask us about support, benefits, services, pay, areas of delivery, residential services, domiciliary care and many more. Please don’t hesitate to ask us a question.

We offer a variety of services throughout Nexxus Care, our three
main areas of delivery are:

Domiciliary Care – Traditional community care services in your own home.

Extra Care – Care delivered within the scheme.

Residential Care – Care delivered within a care home.

Reablement Services – Intensive program of up to 6 weeks following a period of hospital admission to regain independence and prevent future hospital admission.

Learning Disability Respite Care – Respite care is a short-term package of care & support along with accommodation that is offered to people with learning disability’s.

We are commissioned to deliver a reablement program across East Staffordshire where individuals that have had a period of stay in hospital are offered up to 6 weeks intensive program to regain independence. We see reablement as a ‘do with’ service rather than ‘do for’. This service has occupational therapists input alongside highly trained and skilled team leaders/support workers. Please see below what can be expected:

1. Nexxus Care receives a referral from the hospital discharge team. Where we can accommodate safely and effectively Nexxus accept the referral.

2. A senior team leader will arrange to either meet you at your home following discharge or shortly after to agree and produce a personalized care plan. This care plan will consist of any needs our support workers need to be aware of and to produce goals that you want to achieve during this program.

3. Our team will work closely with you, with support from an occupational therapist endeavor these goals are achieved, this may be with the use of assistive technology, for example. Our team continually assesses progress on the program, should the team, the service user, and OT be confident the goals can be achieved the reablement program can be completed and on average, this is within 28 days.

4. If further needs are identified, we have processes to follow to inform the local authority who then arrange longer-term options in the community — such as domiciliary care.
